Clinic Dates and Location
Tuesday, October 13 in the Memorial Union Ballroom from 8:00 a.m.-3:30 p.m.Tuesday, October 27 in the Memorial Union Ballroom from 8:00 a.m.-3:30 p.m.Fred Meyer pharmacy staff will assist with the clinic. The process should take under 10 minutes if participants complete the online form process, including the insurance section and active pharmacy billing information. Fred Meyer recommends waiting on-site for 15 to 30 minutes following vaccination.
Registration Information and Instructions
The October 13 clinic scheduler will close on October 10 at 6:00 p.m. Registration for the October 27 clinic will open after the October 13 clinic. Employees and students will be notified the scheduler is live via email from Employee PEBB Benefits.
To successfully make an appointment, you will need the following information:
Registration link: Fred Meyer Vaccine Sign-Up SchedulerBenefit code (in ALL CAPS): OSUVAC26
Detailed instructions, screenshots on the registration process, and information on eligibility and other options for vaccinations are available on University Human Resources’ Vaccine Clinics webpage.
Insurance Information
Participants must bring their insurance cards to their appointments. If Fred Meyer does not have or cannot obtain correct billing information prior to the clinic, they will be unable to provide your vaccine(s) until they have received payment from your insurance provider, and this will cause a significant delay to your vaccine appointment.
Once you sign up for an appointment, Fred Meyer may begin billing/processing the vaccine(s). If you decide to get your vaccine at another pharmacy prior to the clinic, there may be billing issues as Fred Meyer may have already billed for the claim.
Vaccines are billed out of the Bend, OR Fred Meyer pharmacy; if you receive a notification that a prescription is ready for you, it is likely the vaccine. You can disregard this notification.
Kaiser Members
PEBB members enrolled in a Kaiser Permanente health plan can attend OSU work site vaccine clinics for a flu vaccine. However, if you are enrolled in Kaiser Medicare or Kaiser Medicaid, vaccines are not covered at this site. For participants with Kaiser insurance who plan to receive the flu vaccine at an OSU clinic, it is important that you include your Kaiser Member ID when entering your insurance information in the scheduler. This helps ensure claims are billed correctly and allows the pharmacy team to plan appropriately for the number of Fluzone vaccines needed at the clinic.
For more information about other Kaiser flu shot options, use the Kaiser flu shot clinics locator tool.
Additional Options
PEBB medical insurance plans cover vaccines at many pharmacies or at medical appointments. PEBB’s Vaccine Coverage webpage provides more information.
